Cancelling an Insurance Policy for an Operator’s Certificate Filing
An Insurance Representative or Insurance Broker can cancel an insurance policy for a certificate filing by opening the Insurance Policy List screen from the Insurance Policy menu item, selecting a list row for a certificate with an “Active” or “Precert or Proposed” status, and clicking the Cancel Policy button.
To Cancel an Insurance Policy for an Operator’s Insurance Certification Filing:

Click the Insurance Policy menu title.
The system displays the Insurance Policy List screen with the Policy List grid listing existing policies for the associated insurance company. The list displays data within columns as follows (from left to right): Air Carrier, Certificate Number, Policy Number, Blanket Policy, Coverage Type, Liability Type, Coverage Amount, Filing ID, Insurer Name, Submitted Date, and Policy Status.

Using the Insurance Policy List screen, select the policy to be cancelled. Note: Only a policy for a certificate filing with an “Active” or “Precert or Proposed” status can be cancelled.

Click the Cancel Policy button, located above the Policy List grid.
The screen displays a header with the text “Cancel Insurance Policy for” and the operator name, with the Form ID (example “6410-2023-14007”) and, below this, field groups for the company’s Contact Information, Effective Date, and Submit.

Use the fields and controls to enter data for the policy cancellation.
Contact Name (display): Displays the name of the company point of contact’s (POC’s) Last Name and First Name.
Title (Required): Enter the POC’s title (i.e., business position) within the company.
Insurer Contact (If different than Submitter): Allows entry of an alternate POC for the insurance policy cancellation action.
Effective Date (Required): Use the date field and/or calendar control to select an Effective Date for the Cancelation (Note: The date must be at least 10 days later than current date).
Submit check box (Required): Click the Submit check box to complete the insurance policy cancellation action and that you are “an authorized representative of the insurer or broker, who is authorized to sign/submit form”.
Comment: Use the large Comment field to enter text describing the cancellation action (Allows entry of up to 1000 characters).

Click the Submit button to submit the policy cancellation to the FAA (Note: While the policy’s status is Pending Cancellation, an authorized user can back-out the policy cancellation action by using the Policy List to select the policy and then clicking the Undo Cancellation button. However, after cancellation has been processed by FAA staff, the system will not allow further changes to the insurance policy for the certification filing.
The system redisplays the Insurance Policy List screen with the Policy List grid row now reflecting the pending cancelled policy (The row’s Cancellation Status column displays the “Pending Cancelled” status). The system will generate and send the designated point of contact an automated e-mail indicating the “Notification of Pending Policy Cancellation” within the subject line. Once the policy cancellation is processed by FAA staff, the policy will be included in the Cancellation History screen’s Cancellation History List grid.
